1. Acceptance of these terms

By visiting this website, creating an account or using our services, you agree to these Terms of Use and to our Privacy Policy. If you do not agree, please do not use the website or the services.

Our Notice of Privacy Practices explains how your health information is protected.

2. Who may use our services

You must be at least 18 years old to create an account and receive care through this website.

Telehealth visits are available only when you are physically located, at the time of the visit, in a state where our provider is licensed. We currently serve patients located in Florida. You will be asked to confirm your location, and you agree to give us accurate information.

3. Our services and the limits of telehealth

Integrated Health Clinic offers telehealth services led by Ezequiel Dishmey, AGACNP, MSN, RD. These include a GLP-1 health questionnaire, appointments, secure messaging with your care team, and visits that may be billed to insurance.

Telehealth is not suitable for every condition. After reviewing your information, the provider may recommend an in-person evaluation or decide that a service is not appropriate for you.

Completing the questionnaire or making a payment does not guarantee that you will be accepted as a patient or receive a prescription. Your provider makes that decision after reviewing your health information.

This service is not for emergencies. If you believe you are having a medical emergency, call 911 or go to the nearest emergency department.

Prescriptions are issued only when the provider determines they are medically appropriate and permitted by law. Controlled substances are not prescribed through telehealth.

4. Your account and security

You create an account with your email address and a password. You are responsible for keeping your password confidential and for activity that takes place under your account.

Please tell us right away if you believe someone has used your account without your permission. We may suspend an account when needed to protect patients, our care team or the security of the service.

5. Payments and refunds

Payments are made in U.S. dollars through Stripe Checkout. The amount and the plan you are purchasing are shown before you pay. Stripe processes your card details; we do not store card numbers.

Refund requests are reviewed individually. If you have a question about a charge, please contact us using the details at the end of these terms.

If your visit is billed to insurance, coverage, copayments and any self-pay fees are confirmed before or at the time of your visit. The insurance plans we accept may change.

6. Appointments and cancellations

You can request, view and change appointments through the options the platform provides. If you need to cancel or reschedule and cannot do so in your account, contact us as early as possible.

We may need to reschedule an appointment because of provider availability or clinical needs. When that happens, we will contact you with new options.

7. Acceptable use

You agree to use the website and services lawfully and in good faith. You agree not to:

• provide false or misleading information, including about your identity, health history or location;
• use another person's account or share your account with others;
• attempt to gain unauthorized access to the website, other accounts or our systems, or interfere with their operation;
• upload files that contain malware or that you do not have the right to share; or
• use the messaging feature to harass or threaten our staff or others.

8. Intellectual property

The text, graphics, logos and other content on this website belong to Integrated Health Clinic or its licensors and may not be copied or reused without our written permission. Names of third parties, including insurance plans, belong to their owners and are used only for identification.

9. Medical disclaimer

General information on this website is provided for education and does not replace individual medical advice. A provider–patient relationship is established only when the provider accepts you as a patient. Always follow the guidance of your provider, and do not delay seeking care because of something you read on this website.

10. Third-party services and links

Some features are provided by other companies, such as Stripe for payments and CharmHealth for insurance visits and electronic health records. Their services may be subject to their own terms. We are not responsible for websites or services we do not operate.

11. Limitation of liability

The website is provided on an "as is" and "as available" basis. To the extent permitted by law, Integrated Health Clinic is not liable for indirect, incidental or consequential damages arising from your use of the website, or from interruptions or errors in the service.

Nothing in these terms limits liability that cannot be limited by law, and nothing in these terms limits your rights regarding the professional medical services you receive.

12. Governing law

These terms are governed by the laws of the State of Florida and applicable federal law, without regard to conflict-of-law rules.

13. Changes to these terms

We may update these terms from time to time. The effective date at the top of this page shows when they were last changed. If we make a significant change, we will take reasonable steps to let you know, such as a notice on the website. Continued use of the services after a change means you accept the updated terms.
