Effective date: October 22, 2026. Integrated Health Clinic complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability or sex.

1. Our commitment

Integrated Health Clinic does not exclude people or treat them differently because of race, color, national origin, age, disability or sex, including sexual orientation and gender identity. This notice is provided in accordance with Section 1557 of the Affordable Care Act.

2. Free aids and services

We provide the following at no cost to you:

  • aids and services for people with disabilities, so that they can communicate effectively with us, such as qualified sign language interpreters and written information in other formats (large print, audio and accessible electronic formats); and
  • language services for people whose primary language is not English, such as qualified interpreters and information written in other languages.

3. How to file a grievance with us

If you believe that we have failed to provide these services or have discriminated in another way, you may file a grievance with our Section 1557 Coordinator by phone, email or mail. We will review your concern and respond to you. If you need help filing a grievance, we can help you.

4. How to file a complaint with the federal government
