1. This website is for information only
The content on this website is provided for general informational purposes about the services offered by Integrated Health Clinic. It is not medical advice, does not create a provider–patient relationship, and is not a substitute for a consultation with a licensed healthcare professional. A provider–patient relationship is established only after you complete registration in our patient portal and a visit is conducted.
2. Not for emergencies
Do not use this website or telehealth services for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.
3. Telehealth eligibility and state licensure
Telehealth services are available only to patients who are physically located, at the time of the visit, in a state where the provider holds an active license. We currently serve patients located in Florida. You will be asked to confirm your location before each visit. Telehealth is not appropriate for every condition; the provider may determine that an in-person evaluation is required.
4. Prescriptions
Prescriptions are issued only when medically appropriate, within the provider's scope of practice and in accordance with applicable state and federal law. Controlled substances are not prescribed through telehealth. There is no guarantee that any particular medication will be prescribed as a result of a visit.
5. Scheduling, insurance and payment
Appointments are booked through the CharmHealth patient portal, which has its own terms. Insurance coverage, copays and self-pay fees are confirmed in the portal before your visit. Listed insurance plans may change without notice.
6. Intellectual property
All text, graphics, logos and images on this website are the property of Integrated Health Clinic or their respective owners and may not be reproduced without permission. Third-party names, including insurance plan names, are the property of their respective owners and are used for identification only.
7. Third-party links
This site links to third-party websites, including our patient portal. We are not responsible for the content or practices of those sites.
8. Disclaimer and limitation of liability
This website is provided "as is" without warranties of any kind. To the fullest extent permitted by law, Integrated Health Clinic is not liable for any damages arising from your use of, or inability to use, this website. Nothing in these terms limits liability that cannot be limited by law, including liability for professional medical services provided during a visit.
9. Governing law
These terms are governed by the laws of the State of Florida and applicable federal law of the United States, without regard to conflict-of-law principles.
